Forms for Annual Returns
The Commandery Forms Packet and Membership Forms below are required to be completed and returned to the Grand Recorder AND Inspecting Officer no later than January 15th. Please do not use the deprecated spreadsheets, which have been removed from the website. Filling out the Commandery Forms Packet from the top down will auto-fill many of the subsequent fields. You may email the forms (preferred), or send them by post. Remember, if you do not file these forms by the prescribed dates, per Article 344 of the Statutes and Regulations, your commandery will not be eligible for any merit awards, and you may lose voting privileges at the Annual Conclave.
